Zusammenfassung

We present a clinical case of an upper incisor, abutment of a prosthetic restoration, showing symptoms that were initially diagnosed and treated as pulpar etiology. After a short period of improvement the symptomatology increased and when retreatment was attempted it was evident the complexity of the case. Finally the decision was to remove the ceramic restoration and extract the tooth. The initial macroscopic inspection confirms the existence of a palatine developmental groove running until the apex, origin of a periodontal pathology hardly curable. When the tooth was sectioned and observed under a stereoscopic microscope it was proved a high complexity of the radicular anatomy including a second canal extremely thin and impossible to instrumentate. The combination of radicular groove, periodontal pathology and a second canal are the sources of the treatment failure in this lateral incisor.
